What does this mean in practise?

Working as an integrated delivery partner, we help clients improve outcomes by setting a clear vision and ambition, designing and guiding the transformation journey, and applying both existing and new technology in meaningful, purposeful ways. Crucially, we work with you to develop and embed new models of care and support that this technology makes possible.

We believe real transformation only happens when digital and data are not bolt-ons but are fully embraced to enable different ways of working - and better care for the people who need it.

We’ve captured our thinking in a Digital Blueprint for Social Care - a practical and ambitious vision for what the future could look like. It sets out how, when strategically applied, digital and data can drive better outcomes for people, unlock productivity across the system, and foster a more empowered, satisfied workforce.

Our Blueprint is a practical roadmap for a new model of Social Care - one that we are starting to build today.

How do we deliver this in Social Care?

The complex challenges facing social care are often themed into four critical areas of demand, finances, workforce and the market.  At Channel 3, we recognise that every client has different challenges and unique characteristics, and we use our insight lens to ensure that any work we deliver will have the maximum impact for our clients.   • In our everyday work, we see rising levels of demand and complexity, workforce pressures, limited funding, and shifting expectations from people, families, and carers. But we also see cause for optimism. There is growing momentum behind the idea that adult social care can be reimagined - not in the distant future, but today.

    Our digital blueprint sets out how that future can be delivered across the person’s journey – from digitally enabled prevention in communities through to technology-enabled assessment and long-term support - and through a fundamental step-change in the social care operating model. In this future model of care, digital and data free up capacity, support more personalised and proactive care, empower the workforce, and drive better outcomes at more sustainable levels of spend.

  • Case management systems are at the heart of adult social care — shaping how practitioners work, how information flows, and ultimately, how people experience care and support. But too often, these systems create friction instead of enabling better practice.

    We work with clients to transform their CMS into tools that empower social workers and frontline teams, support strength-based and person-centred practice, and unlock better outcomes for people. Whether selecting a new system or rethinking how an existing one is used, we help organisations choose, design and embed solutions that are intuitive, flexible, and fit for the realities of care today.

  • CHC is facing increasing pressure from rising demand, growing complexity of need, and fragmented processes across systems. In many areas, inconsistent approaches to assessment, case management, and brokerage are driving up costs and creating variation in outcomes.

    We partner with health and care systems to drive transformation in the way continuing healthcare services and support are delivered that aim to deliver better outcomes for people and greater value for the system. This spans the entire operating model - from standardising CHC processes across a locality or region, to consolidating all partners onto a single client management system, to improving market and price management through the adoption of e-brokerage solutions aligned with health and care partners.
